How it works
A short, structured form captures context and consent — no access to clinical notes beyond the programme.
Remind's programme lead reviews each referral and pairs you with a trainer and a gym that fits.
Baseline in week one, structured sessions through weeks two to seven, endline in week eight. A wellbeing check every session.
Outcome reports flow back to the referring GP only after review by the Remind coordinator. Trainers never communicate with GPs directly.
